Information Needed:
- Letter requesting our wholesale information and the name of your business, street address,
city, state, and zip. Please tell us a little bit about your business and your customer base.
Remember to include your phone number, fax, and e-mail address.
- Copy of your state-issued business license
- Copy of your state-issued resale certificate (business license and resale certificate my in
fact be one in the same.)
- Picture of your storefront
- Copy of your yellow page advertising bill
